Among the different agency types, for-profit nonchain agencies served the highest proportion of dual eligible beneficiaries (42.2%) and African-Americans (27.9%) in 2018. On the other hand, chain HHAs had a higher star rating, indicating better quality of care on self-reported process measures and risk-adjusted self-reported outcome measures. However, they performed worse on risk-adjusted claims-based outcome measures.

The Southeastern states, such as South Carolina, Alabama, and Tennessee, had the highest percentages of Medicare enrollees receiving home health services from chain providers in 2018. In fact, the number of Medicare enrollees served by chain agencies increased substantially nationwide, with only one state (Montana) not having enrollees served by any home health chains in 2018.
