From 2005 to 2018, the number of home health agencies (HHAs) has seen substantial growth. According to a study published in PubMed, the number of agencies increased from 7,899 in 2005 to 10,818 in 2018. During the same period, chain-owned HHAs doubled in number from 903 to 1,841.

For-profit nonchain agencies were the largest category in 2018, representing 67.8% of all agencies and serving 40.7% of Medicare enrollees. For-profit chain agencies experienced the most growth in the number of Medicare enrollees served, increasing from 439,998 to 1,082,385.

Among the different agency types, for-profit nonchain agencies served the highest proportion of dual eligible beneficiaries (42.2%) and African-Americans (27.9%) in 2018. On the other hand, chain HHAs had a higher star rating, indicating better quality of care on self-reported process measures and risk-adjusted self-reported outcome measures. However, they performed worse on risk-adjusted claims-based outcome measures.

The Southeastern states, such as South Carolina, Alabama, and Tennessee, had the highest percentages of Medicare enrollees receiving home health services from chain providers in 2018. In fact, the number of Medicare enrollees served by chain agencies increased substantially nationwide, with only one state (Montana) not having enrollees served by any home health chains in 2018.

In 2018, for-profit chain-owned HHAs served a much larger number of Medicare enrollees compared to for-profit nonchain HHAs. The top five for-profit chains served approximately 675,000 Medicare enrollees, while the top five nonprofit chains served around 64,000 Medicare enrollees.

When it comes to quality of care, chain-owned HHAs, both for-profit and nonprofit, had the highest average star ratings in 2018. These agencies performed better on self-reported process and outcome measures. However, nonchain agencies performed better on claims-based outcome measures [9].
